The Moore County Health Department is seeking input from the community.

An opinion survey with five questions is currently posted on the health department website. The short survey takes only a few minutes to complete and is part of the department's accreditation process through the N.C. Local Health Department Accreditation Board.

"We want to assess the services our agency offers and gather information on other services our citizens may want our department to provide," said county Health Director Robert Wittmann.

The survey will run through July 20.

Residents are encouraged to complete the survey on the department's website, www.moorecountync.gov/health. Hard copies are available at the following departments:

n Animal Center, 5235 U.S. 15-501, Carthage

n Environmental Health, 1042 Carriage Oaks Drive, Carthage

n Health Depart-ment, 705 Pinehurst Ave., Carthage
